This report presents the findings of the biannual Maine Health Access Foundation Site Self-Assessment Survey (MeHAP SSA Survey), which is designed to show the current status of a practice’s bidirectional integration to encourage conversations among integrated care team members about their current status and where they would like to be in the future.
